Well, Operation Desert Storm was the Gulf War right? So what is Operation Iraqi Freedom going to be called? Gulf War II?
My friend, Alex, suggested
Gulf War 2: America strikes back.
It probably will end up being known as the Second Gulf War. Although it may simply be reffered to as 'Iraq', the same way Vietnam is just referred to by the name of the country.

That's how the phrase originated, but it can mean any phrase where you say something else accidently.

Well, it's how the phrase is usually taken, but in a more general sense it refers to an individual's true thoughts being revealed by a slip of the tongue.
