we could always disqualify Janeway on the basis of prelimitay drug testing. They found stupid steroids in her urine.
In the beginning the Universe was created. This has made a lot of people very angry and been widely regarded as a bad move.

Even if Janeway is the runaway winner, it'll still be interesting to see which people consider the worst moment for each individual.
Only two things are infinite - the universe and human stupidity, and I'm not sure about the universe: Albert Einstein.

I don't see why we should - there are plenty of other possibilities for her stupidest moment, and I don't see why we should skew the results simply because one of them is a runaway winner. We didn't disqualify Spock's death as the "greatest first officer moment", despite the fact that it was a foregone conclusion that that would come out on top.

Kirk: TWOK. Sitting around doing bugger-all when the Reliant was closing under comm silence.

Picard: Can't think of anything that really stands out, but not heading home when Guinan advised him to in "Q Who?" was far from sensible.

Sisko: Another tricky one. Probably warning the Cardassians about the planned Klingon invasion.

Janeway: Take your pick. You could probably find at least one and possibly several in every single episode.

Archer: Another case of lots of stupidity, but none I can think of that really stands out.
